Parental consent form

Please submit the following application to the right in order to give your child permission to participate in Urban Advocates Foundation SEL Lunch/Recess Program. You are also open to being contacted about community resources. This program provides opportunities for students to develop important social-emotional skills, such as empathy, problem-solving, and self-awareness.
